This part of the business has been in existence since the early 1990's. South Yorkshire Housing Association are the owners of buildings, the landlords and the registered owners responsible for the provision of care within the homes. However the staff providing the care and support in these homes are working on behalf of SYHA but are not employed by the Association. The staff are employed by either Care Trusts or by Social Services hence the partnership arrangements in place for delivering these services.

Abbeyfield Scott Road, Burngreave - Part of SAMS
Support providers - Sheffield Care Trust
8 beds, learning disabilities, supported living scheme
This scheme comprises 4 properties with accommodation for 3 wheelchair users. 24 hour staffing offers consistency and individual quality of care. We offer support for adults with learning disabilities. Our aims are based on choice, respect, dignity, privacy and independence. Staff help with a range of basic life skills and occupation and leisure activities. Both sites have large private gardens with a patio and provide a relaxed, friendly, and caring atmosphere.

Beighton Road, Woodhouse

18 beds, learning disabilities, registered care home
Beighton Road is a Registered Care Home accommodating people with learning disabilities in single bedrooms in 3 separate houses, with 24 hour support. The open design of the site allows residents to express themselves without presenting a risk to themselves or other people. We aim to provide a high standard of care in a homely environment. We treat all occupants with the necessary dignity and respect, allowing them to develop as individuals.

Birch Avenue, Chapeltown

40 beds, elderly mentally infirm (EMI), registered nursing home
Birch Avenue is a nursing home for older adults suffering from dementia. We aim to provide a high quality of nursing care delivered by a skilled and caring nursing and support team in a homely environment. We provide a range of social activities, which involves occupants and visitors.

18 beds, learning disabilities, registered care home
A Residential Care Home for people with learning disabilities with accommodation suitable for some people with physical disabilities. We aim to provide and maintain a quality service of care delivery from a multi skilled and committed care team. We offer a homely environment where we respect individuality, independence, sexuality and privacy. Our aim is to promote community awareness and acceptance.

East Bank Road, Arbourthorne
In partnership with Sheffield Care Trust
20 beds, learning disabilities, registered care home
There is 24 hour support and we aim to recognise, maintain and develop each person's individuality; to respect their dignity and show them courtesy. The needs of the individual are assessed and a care plan devised. We aim to provide an informal, friendly and homely atmosphere while providing as much as support as required.

Grange Crescent, Sharrow

40 beds, older adult, elderly frail, registered care home
Grange Crescent, in the Sharrow area, is a residential home for the elderly. We have 40 single en-suite bedrooms, in living groups of 10, so the occupants are in small groups. The staff aim to provide the highest individual care possibilities, respecting rights, choice, independence, dignity and fulfillment.

Midhurst Road, Foxhill

Midhurst, in the Foxhill area, is a residential home for the elderly. There are 40 en-suite bedrooms, in clusters of 10 so the occupants are in small groups. The staff aim to provide the highest individual care possibilities, respecting rights, choice, independence, dignity and fulfillment.

We strive to provide the best possible environment and support service to the occupants of the home on an individual basis, encouraging them to take responsibility for their lives both on a daily basis and long term planning, through involvement and participation, creating a growing confidence and self determination.

Cranworth, Eastwood
In partnership with Rotherham Care Trust
24 beds, learning disabilities, with residential and nursing care
Our aim is to provide a high quality comprehensive service, where the development of the individual is the aim. We will seek to maximise the well-being of each individual by the use of a planned systematic and regularly reviewed care plan and by working in an active partnership with all other carers and agencies, as appropriate. There are a number of fully adapted ground floor rooms, and one house has a lift to assist residents whose mobility is limited. Now provides nursing care.

John Street, Thurcroft
In partnership with Rotherham Care Trust
15 beds, learning disabilities, registered nursing care
John Street is home to 15 people with severe learning and/or physical disabilities, accommodation in 3 purpose built bungalows. We provide an individualised service delivery complete with a total commitment to quality and enablement. The primary aim is the development of the individual based upon a right to respect choice and confidentiality.

Oak Close, Wath

18 beds, learning disabilities, nursing and residential care
The purpose of Oak Close is to provide housing with care for people with a learning disability to enable them to live in non-institutionalised life as part of the community. We aim to provide a caring environment while promoting as independent a lifestyle as possible for each resident within the home.

Gardens Lane, Conisborough
In partnership with Doncaster Healthcare Trust
12 beds, learning disabilities, registered care home
2 purpose built residential care home bungalows with 24 hour staffing cover provided. Each bungalow houses 6 occupants with learning disabilities. The aim is to provide care and support in a non-institutional setting and to create opportunities for each individual to maintain and acquire new skills. This is to be achieved by encompassing occupants' choice, wherever and whenever possible.

Howbeck Close, Edlington
In partnership with Doncaster Healthcare Trust
14 beds, learning disabilities, registered care home
The bungalows are purpose built dwellings providing a homely environment and the method of care delivery supports the idea of normal living. The philosophy of the home is to provide care for each individual which is based on their personal needs and choice.

Larch Avenue, Finningley
In partnership with Doncaster Healthcare Trust
10 beds, learning disabilities, registered care home
Larch Avenue is home to 8 individuals with learning disabilities. We aim to maintain dignity and privacy by recognising individuality and enhancing decision making. We have 24 hour staffing and provide a homely atmosphere with a caring and trusting relationship with our occupants. We actively encourage them to participate in community activities and to be independent.

Sandringham Road
In partnership with Doncaster Healthcare Trust
6 beds, learning disabilities, registered care home
A purpose built bungalow for 6 people with learning and physical disabilities providing a homely environment which aims to maintain current skills and to develop new skills. We promote choice and individuality, encourage independence and give greater life experiences. We ensure the principles of normalisation are adhered to and each resident has a named nurse and a special interest person to ensure that the above are adhered to.

SAMS - Partnership Schemes
SAMS (Support and Management Services) provides a service to residents in a number of local authority areas. These residents have 'Support Providers' who also assist them. Some of these support providers are social services and others are voluntary organisations. SAMS has clients who have learning disabilities, mental health problems or physical disabilities. All clients who live in our properties are South Yorkshire Housing Association residents.
