Doncaster Social Prescribing
Doncaster Social Prescribing is an innovative project that gives healthcare professionals in Doncaster the option to prescribe non-medical support to patients. We’re working alongside Doncaster CVS to deliver this project and ensure that everyone in Doncaster has access to the best possible support for their individual needs.
Following a successful pilot in 2014/15 where we worked with 29 GP practices in the area we’ve now been commissioned by NHS Doncaster Clinical Commissioning Group and Doncaster Council to roll the project out across the borough.
Doncaster Social Prescribing gives GPs, pharmacists and community nurses the option to refer patients for non-medical conditions such as loneliness, housing or advice on debt.
Our Social Prescribing Advisors then visit patients who are referred to the project at their home and assess their individual needs. We’ll then put them in touch with local services and community groups that can help them with their specific needs.
Making a real difference
By providing support on non-medical conditions Doncaster Social Prescribing has been proven to reduce unnecessary GP appointments and hospital admissions.
In December 2016 Sheffield Hallam University produced an evaluation report of the service’s first year of operation. Between August 2015 and July 2016 the service receive more than a thousand referrals and enabled over 600 people to access support in their local community.
During this period almost half of the service’s clients demonstrated an improvement in their health related quality of life in the period following their referral to the service, in particular there were significant improvements for clients who presented with anxiety and depression.
The results of the report highlight that more than two-thirds of clients reported making fewer GP appointments following their engagement with the service – helping to ease pressure on local NHS services.
